European LED Academy SessionPalais des CongrèsParis

This session was   dedicated to acne management. Speaker’s lectures try   to emphasize the role of LED in this fieldthe choice to devote an entire session to acne, based on the great suffering disease that stigmatizes a fragile population, teenagers and young adults.Disfigurement felt - effect can make the bed of heavy depression, some even attempted suicide.

Michele Pelletier, Dermatologist , European LED Academy President prevented at the last moment to come to Paris , I was asked to make presentation :

" What does Photobiomodulation (PBM)psychological felt the teenager facing his disgrace ? " Statement whose purpose is illustrated by several clinical cases.

They concern two young patients, 15 and 18 and three young patients, 23, 25 and 30 and serve to emphasize the importance of depressive factor in the initial consultations.

He even true despairs , rejection of any treatment, the fact of blaming medical corps that hears the complaint but did not listen to  , possibly for a patient psychotherapy  . In any event, these five profiles, yet very different, had resulted in treatment failure before being treated "differently" , (This is to say) based on the mechanisms of PBM .

Support chosen by Michèle Pelletier was crowned with success effect , in all cases , a very significant improvement quality of life : Disappearance of body dysmorphic disorder , patient reconciliation with himself and of course very important improvement cystic lesions .

Michèle Pelletier was able to highlight the benefits of direct healing skin lesions Photobiomodulation (PBM) but also psychological benefits , directly related to regular exposure to light sources in the fields of red and near - infrared.

In practice, the treatment protocols used were focused on LED sessions . These have associated different wavelengths, in the areas of blue , yellow, red and infrared.

For a patient is ambulatory LED device that allowed him to be treated at home , in this case at a rate of 2 sessions per week for 3 months.

In 2 cases, several sessions of intense pulsed light (IPL ) were associated .

In 2 other cases peeling sessions called «Photodynamic " were used. They consist of applying before light  , LED topical ( preparation officinale ) based niacinamide , dimethyl isosorbide , iron oxide and zinc.

The briefing by Michèle Pelletier ended with the presentation of LED devices combining different wavelengths in the blue, red and infrared , and even headphones with MP3 player into the idea that synergy between phototherapy and Music Therapy ...

She finally concluded this difference mentioned above, by proposing the concept of " acne coaching " that binds purpose :

• improve the disfigurement by coupling LED peels and IPL

• manage stress by a small support psychotherapeutic and light therapy

• the " renarcissiser " the patient by showing him photos support, it gets better ,

• the " involved" through ritual sessions (2 per week for 6 weeks).

The presentation of François Michel, Dermatologist, then, gave an update on «clinical acne aspects."

He dealt with globally starting with making this disease a particular data of our  civilization fewer and fewer tolerated ;involving 90% of people and appearing increasingly early in life .

After a review of its pathophysiology and its various clinical forms, he stressed that in acne, the couple infection / inflammation plays a key role that could be fought by the biological properties of light.

Indeed, in the field of blue (400 to 475nm), the light destroys P.acnes , activating its own porphyrins . In the field of red to near infrared (600 to 900nm) it develops anti- inflammatory properties.

Illustrated by an iconography of high quality, several clinical cases and allowed us to explore the efficiency of the LED monotherapy.

He then through epidemiology, emphasized the role of this devastating disease on quality of life, may, among adolescents, lead to anxiety and depression syndromes heavy and suicide.

He also emphasized the etiological role superimposed comedogenic cosmetics, cleaning skin too incisive, excessive consumption of fast sugars, or the role of certain drugs.

He considered all available treatments emphasizing the ease offered by LED Phototherapy in blue and red, in addition to maintaining good local hygiene and a healthy lifestyle. He recalled that once cured, the acne disease could leave scars and may tarnish the quality of life forever.

In conclusion, treatment now and prevention need for future, make this condition should always be taken seriously and considered through all the parameters that determine it.

Finally, about the role of photobiomodulation brings us together; he suggested a broader and more accessible LEDs use, offering us to reflect on the role that the home looks very promising device.

In summary, treatment initiated and controlled by the practitioner and pursued effectively at home.
